The Behavioral Health Nurse Care Manager works in conjunction with multidisciplinary teams to coordinate and provide interventions to improve quality of care and health outcomes for clients, to support integrated care efforts, and ensure program compliance with required health metrics.
DUTIES:
- Initiate, engage, assess, monitor, educate, coordinate, and provide interventions for participants and families to improve quality of care and health outcomes.
- Provide clients and family/caregivers teaching and education about health and wellness, safety factors, self-management, medications, self-monitoring, action plans, long-term planning, and how to access care.
- Participate as a member of a multidisciplinary team by attending client meetings, consultations, internal team meetings, and external stakeholder meetings.
- Assist with the development, implementation, and ongoing monitoring of Individual Service Plans by participation in client meetings, outreach to clients and caregivers, collaboration with healthcare providers, and making referrals.
- Ensure program compliance with required health metrics by monitoring measures required by the state and connecting directly with clients, families/guardians, and team members to educate and problem-solve barriers.
- Communicate with clients' PCPs upon admission and as needed to gather information pertaining to physicals, medications, health conditions, and to discuss any significant healthcare needs or gaps in care.
- Review and monitor HIN and VMS for pertinent participant health information and update this information in the Spurwink electronic health record.
- Monitor participant Emergency Department (ED) visits and provide follow-up intervention and consultation. Facilitate ED diversion planning for participants identified as high ED utilizers.
- Review participant health questionnaire data to identify and address individual and population health needs.
- Maintain required records and documentation.
QUALIFICATIONS:
Must be a registered nurse; a psychiatric nurse licensed as a registered professional nurse by the state where services are provided and certified by the American Nurses Credentialing Center (ANCC) as a psychiatric and mental health nurse; a PMH-APRN who is licensed as a nurse practitioner or clinical nurse specialist by the state where services are provided, who has graduated from a child and adolescent or adult psychiatric and mental health nurse practitioner or clinical nurse specialist program and is certified by the appropriate certifying body; or an advanced practice nurse, as defined by the Maine State Board of Nursing.
