Along with a competitive starting salary of $22.07/hr, you’ll have the chance to earn performance-based pay increases, reaching up to $24.52/hr within just six months. This is a full-time, 40 hours per week position, with an incredible opportunity for growth.

As a Case Manager, you will be at the heart of making real, lasting changes in the lives of individuals in need. You’ll work closely with clients, providing personalized assessments and connecting them with essential services to help improve their lives. Key Responsibilities:

  • Client Assessments: Conduct thorough assessments to understand clients’ needs, goals, and strengths, using a person-centered approach.
  • Care Planning: Develop individualized care plans with clients, families, and other professionals to ensure comprehensive support.
  • Service Coordination: Help clients access healthcare, housing, education, employment, and other crucial resources.
  • Advocacy: Stand up for clients’ rights and ensure they have access to the services they deserve.
  • Monitor Progress: Track client progress, adjusting care plans as needed to ensure the best possible outcomes.
  • Crisis Support: Provide intervention and support during times of crisis, working alongside emergency services when necessary.
  • Documentation: Maintain accurate, confidential records of client interactions, services provided, and progress.
  • Collaboration: Work with community organizations, government agencies, and others to address challenges and improve service delivery.
  • Stay Informed: Keep up to date with the latest laws, regulations, and best practices in services and homelessness prevention.

Qualifications:

  • Education or Experience: A Bachelor’s degree in social work, psychology, counseling, or a related field, or at least 5 years of relevant work experience (or a combination of both).
  • Interpersonal Skills: Ability to connect with and build trust among diverse populations.
  • Organizational Skills: Strong ability to manage time, prioritize tasks, and pay attention to detail.
  • Tech Proficiency: Experience with Microsoft Office and electronic health record systems.
  • Reliable Transportation: A valid driver’s license and dependable transportation are required for client visits.
