Registered Dietitian - HYBRID

Job Purpose: Oversee the nutritional well-being of our clients by providing medical nutrition therapy and health education services. The ideal applicant has experience providing nutrition education to patients and mitigating risk factors. The successful candidate will have a proven track record of conducting nutrition assessments and creating appropriate nutrition plans accordingly. Must be willing and able to see clients in a hospital and/or clinic setting if necessary. Must have a valid driver's license and be able to drive the AHN company car when needed.

Duties/Responsibilities:

• Client Support: Assess client's needs and collaborates with the team to develop an individualized education plan of care to facilitate the development and application of needed skills to positively impact disease management. Identify patient's specific disease self-management and medical nutrition therapy goals and develop unique education care plan to best meet individual needs and lifestyle by conducting individual counseling sessions to identify specific self-care.
• Documentation and Reporting: Maintain detailed case notes and provide monthly follow-ups.
• Advocacy and Education: Screen for and refer to appropriate health-related programs and classes. Function as an advocate for the client as needed/when appropriate with program partners, treating physicians/providers, insurance companies, community organizations to ensure the client is receiving the care and resources they need.
• Database Management: Maintain proper notes and documentation through accurate and efficient recording of all pertinent information into the database system. Make sure that all patient records are up to date with the necessary documents and screening tools.

Education/Experience:

• Bachelor's degree in related field from an accredited college or university.
• Must be a current Registered Dietitian - Candidates with a provisional license will be considered.
• Prefer Bilingual in (English/Spanish) but not required.
• Prefer one (1) year of professional experience as a registered dietitian.

Physical Requirements:

• Stand 3-6 hours/day, travel 2x/week, sit at computer/car 4-6 hours/day, lift up to 30 pounds.
