Prescott Home Health is a locally owned and operated agency specializing in adults with developmental disabilities and delivering Physical Therapy, Occupational Therapy, Speech Therapy, and Skilled Nursing services to the Twin Cities metro.

Responsibilities:

This position assists with the planning, coordination, leading, controlling, and evaluating of home health services.  Leads the coordination and management of home health clinical services and acts as a liaison with the medical community, patients, and office personnel. 

Qualifications:

1. Must be a Registered Nurse or professional discipline (Physical Therapist, Occupational Therapist, Speech Therapist, or Social Worker) with at least three years of clinical experience. 
2. Supervisory experience is .
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Solid computer skills; prior experience with electronic medical records (EMR) .
