The Inpatient Case Manager/Discharge Planner is responsible for coordinating and overseeing individual and group case management services and support provided to individuals struggling with behavioral health issues and addiction.
Hours 10am-6:30pm
RESPONSIBILITIES:
- Conduct assessments to determine the needs of clients seeking behavioral health and addiction recovery services.
- Develop individualized treatment plans in collaboration with clients, healthcare providers, and other stakeholders.
- Coordinate care and services provided to clients, including therapy, medication management, and group therapy.
- Monitor and evaluate clients' progress towards treatment goals and make adjustments to the treatment plan as needed.

REQUIREMENTS:
- Valid Driver's License and insurance with clean driving record (required).
- Ability to pass a background check and pre-employment drug screen (required).
EDUCATION:
Required: High School Diploma/GED
: Bachelor's Degree or higher in a Human Services field.
LICENSE:
Required: CDCA
EXPERIENCE:
- 3+ years of experience in addiction recovery or behavioral health settings, with a strong understanding of case management and community resource connections.
