What You'll Do
Comprehensive Patient Care
Conduct in-depth mental health assessments to identify patient needs.
Develop, implement, and evaluate individualized treatment plans.
Provide outpatient therapy tailored to adults and/or children with diverse mental health needs.
Offer crisis intervention services, ensuring immediate support and patient safety.
Collaboration & Community Support
Work closely with interdisciplinary teams to provide holistic patient care.
Establish and maintain relationships with community organizations for patient advocacy.
Connect individuals with mental health and public health resources to enhance their support network.
Monitoring & Reporting
Maintain accurate patient records, treatment plans, and progress reports.
Conduct regular assessments to evaluate patient improvement and adjust treatment strategies.
Prepare detailed case summaries and discharge plans.
What You Bring
Required Qualifications
Education: Masters Degree in Social Work (MSW).
License: Active LMSW licensure.
Experience: At least 3 years of experience working with diverse populations, including expertise in:
Conducting mental health assessments and developing treatment plans.
Providing individual and family therapy.
Crisis intervention and behavioral health support.
Skills & Competencies
Proficiency in evidence-based practices and psychosocial assessment.
Strong treatment planning skills with the ability to adapt plans as needed.
Excellent communication skills for patient counseling and interdisciplinary collaboration.
Ability to handle crisis stabilization and make emergency decisions.
Familiarity with local mental health resources for patient referrals and advocacy.
