JOB DESCRIPTION/QUALIFICATIONS/RESPONSIBLITIES

The ACE Case Manager is a direct care position. The TCM provides Monitoring, Assessing, Coordinating, advocating, referring/linking services and supervision to assist adults with disabilities develop and maintain skills in their home and in their community. Usual hours are scheduled from Monday through Friday, but due to the nature of this work, the TCM may be required to work in the evenings or on weekends. The frequency of visits made by the TCM each week is dependent on the needs of the person served. The TCM reports directly to the Case Management Supervisor.

1. Develop a positive working relationship with administration and staff at ACE and collaboratively develop on-going policies and procedures for effective programming

2. Successfully open new accounts: Meet with referred potential clients, their families, other involved staff to introduce our program and philosophy.

3. Initially assess the appropriateness of the referred individual within 30 days, including conducting a home visit.

4. Identify housing resources available and assist with procurement of same.

5. Assist clients with scheduling of all necessary appointments, coordinating transportation to said appointments, and attending appointments with clients as needed.

6. Provide training in activities of daily living, including but not limited to: personal hygiene/appearance, housekeeping, clothing care, grocery/personal shopping, budgeting, banking, maintaining healthy living, safety and first aid, accessing public transportation, use of community resources, leisure and socialization skills.

7. Meet with the people you serve on a regular and appropriate basis at mutually agreed upon times. (TCM’s are required to see each client at least once per month in the home.

8. Help organize, facilitate and participate in, as appropriate, client recreational, leisure, and social activities. Enhance the integration of the into the community. Work with other ACE staff to coordinate opportunities.

9. Respond to all phone calls (to clients, families, Supervisor, ACE staff/employees etc.) in a timely and professional manner.

10. Initiate contact with other staff interdisciplinary team members and the client’s support network on a regular and appropriate basis. Function as a team member.

11. Assure the provision of a safe environment for clients, staff, and visitors. Consider the safety aspects of household conditions. Respond to emergencies as outlined by ACE policy and in accordance with chain of command.

12. Develop a positive relationship with clients and serve as a role model.

13. Consistently demonstrate an appreciation of the basic human rights of the people we serve and strive to ensure they are treated with dignity and respect.

II. Documentation; Administrative Tasks:

1. Complete a home visit and assessment within 30 days of initial date of service. Update assessment annually. (TCM)

2. Complete a service plan within 30 days. Update service plan every 6 months. (TCM)

3. Complete and submit all required documentation by the end of the day following the service, including progress notes, monthly financial reports, phone contact sheets, medical appointments record form, incident reports etc.

4.. Complete and submit authorization requests (OTRS) upon initiating contact with new clients and opening new cases, maintain unit tracking counts, and submit continuing care requests prior to authorization expiration.

5. Attend scheduled staff meetings, individual supervision meetings, and trainings as requested by Supervisor and Administrative Staff.

Ideal Candidate:

1. Bachelor's Degree in Social Services Field

2. Targeted Case Management Certification

3. 2+ years of case management experience working with people with behavioral health challenges

4. Excellent communication skills (oral and written)

5. Computer Literacy
