Location: Greater Milwaukee
Type: Full-time / Part-time / PRN

Job Summary:

As a Nurse at Wellspring Nursing and Care Management, you will provide a range of services that ensure our clients can receive the highest standard of care in their home or facility setting. Your work will involve performing medical interventions, conducting assessments, managing care transitions, and offering simple to complex care management support. You will work directly with clients and their families to empower them through education, support, and advocacy.

Key Responsibilities:

  • Medical Support: Provide hands-on care, including wound care, catheter care, medication management, and basic assessments.

  • Aging in Place Assistance: Assess client needs, create care plans, and offer support to help them remain safely in their homes, collaborating with families or external caregivers as needed.

  • Care Coordination: Help clients with complex health conditions navigate the healthcare system, access resources, and manage their care plans.

  • How You’ll Do It:

    • Conduct in-home visits

    • Attend doctor and other appointments as an additional support and advocate

    • Coordinate care between clients, families, healthcare providers, and various community resources to ensure seamless care

Ideal Candidate - Qualifications:

  • Education & Certifications:

    • Licensed Practical Nurse (LPN) or Registered Nurse (RN) required.

    • Additional certifications in care management, patient advocacy, or social work are a plus.

  • Clinical Skills:

    • Proficient in basic medical procedures such as taking vitals, wound care, catheter changes. Other advanced skills, like ostomy care, are a plus.

    • Experience in conducting health assessments and developing care plans, especially during transitions between healthcare settings (hospital, home, facility).

  • Care Management & Coordination:

    • Proven ability to navigate the healthcare system, including understanding insurance, financial resources, and available services.

    • Experience in coordinating care among healthcare providers, family members, and community resources, ensuring seamless patient outcomes.

    • Ability to manage complex care needs and support clients through medical transitions.

  • Personal Attributes:

    • Empathetic and patient, with a strong ability to listen and respond to the needs of clients and their families.

    • Organized, proactive, and comfortable working independently with minimal supervision.

    • Flexible and adaptable, able to thrive in an environment where patient needs and priorities can shift.

    • Strong communication skills, both with clients, families, and interdisciplinary healthcare teams.

Additional Requirements/Preferences:

  • Travel Requirements: Ability to travel to client homes, healthcare facilities, and appointments as needed.

  • Availability: Flexible hours, with the ability to adjust based on your and client needs.

  • Geographic Location: Preference for candidates located in or around the southern and western edges of the Milwaukee area.
