Nurse Practitioner (Hybrid/Home Visits) - San Francisco Bay Area, CA

Job type: Full Time/Permanent.

Shift: Monday to Friday, (8 am - 5 pm)

Requirements:

• Active and good-standing Nurse Practitioner license

• Willing to do 5-6 home visits per day in (SF and East Bay)

• Certification in the State of California.

• Active and good standing NPI number.

• Post-graduate training in area of responsibility.

• 1 year as a Clinical/Family Nurse Practitioner.
