1st Choice is searching for a dynamic and passionate Health Program Manager, w/an LMSW, to manage an assessment program serving Medicaid members in the Baltimore City Market.
Supports and implements the Client and Company's mission, vision, and core values within the Assessment Program.

Responsibilities:

• Responsible for strategic, fiscal, regulatory, and operational leadership of an assessment program serving Medicaid members in the state market.
• Accountable for establishing and maintaining collaborative partnerships as part of the contract's account management and vendor management functions.
• Implement and oversee a comprehensive quality assurance program.
• Identify external barriers creating bottlenecks in workload and increased client engagement to solution and risk mitigate barriers,
• Directly supervise remote and field-based assessor
• Serve as the business liaison, including providing detailed requirements and future state vision for a new technology application for the business unit
• Identify available resources by routinely reviewing capacity, productivity, and workload to determine shared services needs to maximize revenue, engagement, and morale.

WORK SETTING AND HOURS: Hybrid; Full-time

LICENSE REQUIREMENTS:

• Minimum Valid LMSW: A valid Maryland license to practice as a professional licensed social worker is required at the time of appointment.
• Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at appointment time.
• All licenses and certifications must be maintained at current levels throughout employment.

EXPERIENCE AND KNOWLEDGE REQUIREMENTS

• Minimum of 6+ years of professional experience providing medical-surgical/geriatric and/or pediatric case management support
• Minimum two years management experience
• Ability to work independently in a home setting to vulnerable Medicaid participants across the age spectrum as needed
• Strong assessment skills
• Experience w/LTSS Maryland highly preferred
• Must possess a high degree of electronic medical record experience/computer literacy and excellent documentation skills.
