Quality Assurance Manager and PCMH

Job Description

Initiates and oversees the development of a comprehensive quality improvement program inclusive of the analysis and trending of data related to initiatives. Must demonstrate integrity, sound judgment, demonstrated leadership skills, and strong interpersonal skills. Must be able to approach staff about quality issues with tact and diplomacy.

Provides overall direction necessary to ensure that clinical services are provided in accordance with standards established through state and federal regulations and AAHCA standards including the National Committee of Quality Assurance that are evidence-based. Must be familiar with PCMH accreditation.

Designs, initiates and facilitates PCMH workflow processes throughout LaSante Health Center. Monitors PCMH compliance. Coordinates training activities related
to PCMH data collection, patient tracking, policy and procedure development, and work flow
changes.
Essential Functions:
1. Oversees the development and implementation of a PCMH recognition program for the
Ingham Community Health Centers.
2. Attains expert knowledge of NCQA PCMH standards, policies, processes and application
requirements. Advises management of changes to continuously improve content and
delivery
3. Develops new policies and procedures necessary for PCMH implementation. Evaluates
existing policies for compliance with the PCMH model.
4. Develops and implements best practices to improve work flow related to PCMH quality
incentives and practice transformations.
5. Develops, oversees and facilitates training programs to align work flow and Health
Department culture with the PCMH model. Provides education and support to staff as they
implement and sustain measures to improve chronic disease management.
6. Oversees the collection, integration and evaluation of diverse data related to clinical and
operational performance and medical home transformation activities.
7. Works collaboratively with other staff members in regard the PCMH model. Evaluates the
performance of care teams and makes recommendations for changes. Provides input to the
care coordination process. Participates in the quality improvement committee to ensure
PCMH issues are included in the overall quality plan.
8. Researches, gathers and reports health related data to meet NCQA requirements. Provides
regular program updates and outcomes to Ingham Community Health Center leadership.
9. Updates documents and participates in the submission process for grants and accrediting
agencies.

Interprets and coordinates compliance with the HRSA Compliance Manual, and other Federal, State and local regulations that are applicable to these practices. Experience working with disadvantaged populations helpful and knowledge of health disparities highly desired. Assist with annual UDS and HRSA reports.

Prepares reports and Assesses compliance with accreditation standards and regulations related to clinical care in collaboration with entity leadership and staff. Identifies areas of vulnerability and directs the development of strategies to enhance compliance.

In conjunction with clinical staff and medical staff directs and coordinates quality improvement initiatives. Assists in the development, monitoring, and presentation of internal quality measures and initiatives. Coordinating tracking and reporting of clinical and utilization outcomes, and follow-up of corrective action plans.

Proactively educates leadership, clinicians, and staff regarding the basic of risk management/patient safety and their role in ensuring that patients are not harmed during care delivery.

In collaboration with clinical staff and service managers, participates in the monitoring, reporting, and improvement activities related to clinical care, health care quality/safety initiatives, accreditation and regulatory requirements.

Participates and coordinates various committees and projects to facilitate the compliance of regulatory and needs of the position responsibilities.

Regularly communicates quality activities to leadership and staff. Outstanding skills in data collection, analysis, and presentation. Experience in the use of spreadsheets (e.g. Excel) for QI-related data management and display (e.g. Tableau).
